An Advanced Practice Nurse Care Manager can perform the necessary physical and mental exams to determine the appropriate level of care including:

•PRJ and Screen

•Mini Mental Status Exam

•Other assessment and diagnostic tests to determine care needs

                                                       

   Determining the appropriate level of care in situations that include, aging at home, assisted living rehabilitation centers, and nursing homes. Further the Advanced Practice Nurse Manage ensures that this plan is maintained.
